Obtaining Home Health Services
You can be considered for home health care services if:
1. Your needs can be met at home
2. You are under the care of a physician
3. You desire to have services in your home
4. You are homebound
5. Payment for home health services when ordered by a physician is arranged through Medicare, Medicaid, Private insurance, Private payment, or worker's compensation
6. PT provided or doctor-ordered
7. ADC provided if have Medicare

- Problem resolution associated with high-risk situation
Other Services
- Nutritional assessments / counseling by a registered dietition
- Services provided 7 days a week
- A registered nurse on call 24 hours a day
